Vein Treatment Q & A

by James Silberzweig, MD

What causes varicose veins?

Healthy veins contain tiny valves that help keep the blood flowing in the right direction - back to the heart - as it circulates throughout the body. Sometimes, these tiny valves can become damaged and stop functioning properly, allowing blood to “back up” and pool inside the veins, creating the bulging, purplish appearance characteristic of varicose veins. Varicose veins and spider veins become more common with age, and they can also occur more frequently in people who are overweight, those who spend long periods of time standing or sitting each day, women who are pregnant, people who smoke and those who take certain medications for long periods of time, including birth control pills.

What symptoms do varicose and spider veins cause?

In addition to their appearance, varicose and spider veins can cause pain, itching, and aching that can interfere with normal quality of life.

How are varicose veins and spider veins treated?

There was a time when the only treatment option for varicose veins was invasive surgery to close off and remove the problematic veins. Today, there are many more options for treatment, enabling each patient to get the best and most appropriate care based on their specific needs. At our practice, we offer several treatment options including:

  • endovenous laser treatments (sometimes called EVLA), a minimally-invasive procedure that uses a tiny fiber to dispense targeted laser energy into large varicose veins, heating, irritating and damaging the vessel walls so the vein closes off
  • radiofrequency occlusion, a minimally-invasive procedure that uses a very thin tube called a catheter to deliver radiofrequency energy to large varicose veins, heating the vein walls can cause them to collapse and close
  • ambulatory phlebectomy uses very tiny incisions to remove the damaged vein without the need for stitches
  • sclerotherapy, which uses injections of special chemicals to scar and irritate vessel walls so they collapse and close off the flow of blood to the damaged vein
  • laser treatment that uses highly focused laser energy to target the vein through the skin without incision or injections, heating the vein walls and causing the affected vein to close

Procedures can be performed using local anesthetics as needed, especially for larger veins.
